Annual Physical & Eye Exam

The Annual Physical Exam should be completed each year and expires one year following the last appointment. Once the physical data has been entered, Magnus Health will send you an alert when the next annual physical is due. 

The Eye Examination Form should be completed for all rising Kindergarten students or new students entering Grade 1 - Grade 5. Students in Fieldston Middle or Fieldston Upper do not need to complete this form.
